It's indicating that the MFA must be done, but nothing's opening/prompting for it. OWA, Windows sign-in, and Outlook mobile app proceed to the MFA registration and/or use. Steps

If you suspect someone else is trying to access your account, contact your IT administrator. The error could be caused by malicious activity, misconfigured MFA settings, or other factors.
